Job Description:

Residential Program The LPN is responsible to assist and support each client through the entire treatment experience from admission through discharge. The LPN monitor's each client in a pro-active manner to ensure that no problem or trouble may compromise the client's or treatment staff's effort. The LPN introduces each new client to his/her fellow clients and the treatment staff, orientates each client to the Discovery Treatment Program, makes the client familiar with the rules, expectations, intent, and routine of the daily treatment schedule; and tours each new client around the building and grounds to familiarize the client with the facility. The LPN determines whether the new client has any special needs and assists the client in fulfilling those needs. Such needs are communicated to the treatment team appropriately.As the Counseling team is available to the clients on a twenty-four hour basis, careful client monitoring and communication with the treatment team is essential. It is paramount that the LPN understands that while the entire treatment staff works as a team, the LPN makes the daily "walk" through the treatment day with the clients. This position requires a strong knowledge of appropriate personal and clinical boundary's, an understanding of eating disorders, a consistently positive attitude, and the ability to deal with difficult, uncertain, and stressful situations in a positive manner while keeping the client focused on their treatment tasks and goals. The LPN consults with the Physician to ensure appropriate medical interventions. The LPN meets with each resident daily to assist with medications, vitals, and weights. The LPN is responsible to carry out any orders received from the Physician and attends treatment team as indicated. Responsibilities:Ensure client's safety through the night by completing bed checks, setting alarm and locking the facility.Remain awake during the night and clocks in each hour.Completes over night duties/responsibilities listed in the change of shift log each evening (including general and specific cleaning duties).Monitor's all client progress through the treatment experience, communicates with the treatment team as indicated.Required to attend and participate in all in-service and educational meetings.Directs clients in a clear, concise, and informative manner.Enters appropriate documentation in the client record after each and every treatment intervention.Greets everyone courteously.Complies with all Center For Discovery policies and procedures.Completes other duties as assigned by the treatment team.Answers telephone promptly and politely, takes complete messages, forwards appropriately.Contributes to the well being of all clients by serving as a positive role model, demonstrating professional and caring manner toward co-workers, clients, and guests.Establishes realistic goals within the position.Familiarizes self with the philosophy, mission, goals, and objective of the Center For Discovery.In the event of illness or emergency that may cause the LPN to miss shift, the overnight must give at least 12 hours notice to the Facility Manager.Makes timely request for leave and adheres to rest break schedule.Maintains confidential nature of all client and program related activities.Performs other assigned and related duties as needed.May be required to travel to other facilities from time to time to attend training's.Implements Physician orders.Dispenses medication as prescribed.Counts narcotics each day, logs appropriately.Meets regularly with medical staff to discuss resident treatment.Provides assessment of medical problems as needed.Performs primary first aid and determines need for physician notification.Is available for twenty-four hour on-call duty.May take vital signs and perform basic evaluative procedures and tests.May provide health education to residents and staff.Responsible to supervise, schedule, educate and train nursing staff.Assists when necessary with medication pick-up.Provides consultation with Forms and Policy committees related to nursing policy and protocol.Attends designated meetings. Education and Experience: LPN certified with some mental health experience. Knowledge: Must have considerable knowledge of nursing principals and practice.Must demonstrate knowledge of medications and their proper application and use.Able to take vital signs.Able to provide basic First Aid.Able to communicate well verbally and written with various members of the treatment team and resident population.Must understand the therapeutic process; understand confidentiality laws, and strong knowledge of professional ethics.Must be able to demonstrate leadership abilities, and be able to work with and communicate with persons from all educational and cultural backgrounds.Must demonstrate tact, initiative, resourcefulness, professional manner, enthusiasm, and a non-judgmental attitude *Neighboring Cities Include: New Haven, Hamden, Newtown, Danbury, Washington, Bridgeport, Norwalk, Ridgefield, Greenwich, Stamford, Westport, Waterbury, Woodbury, Watertown,Cheshire, Commack, Hicksville, Shirley, Bay Shore, Melville, Levittown, Smithtown, Brentwood, Huntington, Stony Brook
